Crime Victim Reparations Brochure
Created by the the Utah Office for Victims of Crime, this brochure provides information for victims who have suffered physical or psychological injury or death as a direct result of criminally injurious conduct and answers frequently asked questions for those who may be eligible to receive reparations for crime related expenses (including family members and dependents of deceased victims or representatives acting on behalf of victims).
Application for Crime Victim Reparations
This application can be used to apply for Crime Victim Reparations ("CVR"). To be eligible for CVR, the crime must have occurred within the Utah jurisdiction or the victim must be a resident of the state if the crime occurred in a location where reparations are not available. Applications can only be considered for crimes that happened on or after January 1, 1987. The crime must have been reported to law enforcement and the applicant must be cooperative with the investigation and prosecution. (Click HERE for Spanish version)
Victim Impact Statement
Victims have a constitutional right "to be heard at important criminal justice hearings related to the victim." This form can be used to provide information to our office and the court about how you have been impacted by the crime(s) committed against you.
